Once the Lancang flows out of China it is called the Mekong.

It rises with the Yangtze and the Yellow River in the 三江源 region of Qinghai. Below Yunnan it becomes the 湄公河, the Mekong, and flows on along Myanmar and through Laos, Thailand, Cambodia and Vietnam to the South China Sea.
